To get a Distinction* on a BTEC course, you need to have achieved a specific number of points (varies with each course) from your different units overall. A singular piece of work can only be awarded a distinction- which is the highest grade.
However, if you get lots of distinctions, your point score goes up and could, in turn, total a Distinction*.
